I was only thinking about removing the silicone and foreign material from my nose, and I told the director that I hated the hard feeling in the columella, did not want ear cartilage placed there, and did not want a columellar strut, so I asked him to remove everything. But the director said that if we did that, the nasal tip could become completely compressed and turn into an upturned nose. At first, he suggested using costal cartilage, but since I said I disliked everything, he said then we should at least place a little bit of ear cartilage for now.
So although it would not provide as strong a supporting effect as a columellar strut, to help prevent it from completely collapsing, he used ear cartilage to keep it from collapsing.
On top of that, even though my septum was weak, septal cartilage had been used in the previous surgery, causing the septum to collapse, so it was in a deviated state. Also, the remaining Medpor had a lot of inflammation around it, and he said there was almost no right alar cartilage pillar left... So he reconstructed it with ear cartilage with difficulty.
So he said he performed a rotational graft using ear cartilage. When I asked what a rotational graft was, he explained that it is a surgery to prevent the nose from lifting upward.
Because I did not want a columellar strut, he also reinforced the columella using ear cartilage with a surgical method? that only that director uses. He said that even if it is not as firm as a strut, it can still provide a certain degree of support.
After surgery, the director said that in a case like mine, costal cartilage should be used, but because I wanted everything removed so much, he used every possible method to create the shape properly, made it like this, and said we should watch the progress for now.
He said that if the scar area on the nasal tip is still severe after surgery, it can be excised and made less noticeable, but first we should reinforce the tissue and watch the progress, and if the scar remains, he will remove it then. Even now, after a lot of time has passed, it does not stand out much, so I am satisfied.
