First of all, I’m aiming for a natural-looking nose shape. My current condition is that the bridge was done with temporalis fascia + crushed ear cartilage, and the tip is septal cartilage. Reason for revision surgery: there is a step-off, and the starting point of my nose is high, so it looks unnatural. I wanted to lower the starting point and also lower the height a bit. + I have experienced silicone inflammation, so I’m not planning to use silicone for the revision surgery. + Among celebrities, I want a Choi Hee kind of look. It’s been almost 15 years since the surgery, so I thought the adhesions would be pretty severe, and I was worried from the bridge removal stage.
1. JS They knew the doctor who did my nose surgery. They had seen him at conferences too, and knew exactly how that doctor performs surgery. So after looking at the CT of my nasal tip, they said that doctor does it this way and so on. Anyway, since they knew the style of the surgeon who operated on my nose, the consultation felt pretty smooth. Since a long time has passed, I thought the adhesions would be severe, but after touching my nose, they said removal seemed possible. However, they said my skin is very thin, so they need to be careful and do the dissection well. They made a plan to reuse the material after removal. If the temporalis fascia + ear cartilage placed high in the glabella area is moved downward, it would resolve both the high glabella area and the step-off in the lower part. The nasal tip and columella shape are not bad, so they would leave them untouched as is. They told me my nasal skin is on the thin side, so the current implant is slightly visible through it. They said we could add MegaDerm or buttock dermis to reduce the show-through a bit. They also explained that if I do alar base reduction + alar rim lifting together, the nose shape would become prettier - this is optional. I forgot to ask whether the celebrity look I’m aiming for (Choi Hee) would be possible;;
2. JY They said they used to do what is currently in my nose too, but it gives the nose a slightly chalk-like? feeling, so they no longer do it. They said that if it’s removed and reused, the impression of something thin and chalk-like inside the nose when viewed from the front would not improve. They seemed not to know the doctor who previously did my nose surgery, and they didn’t ask which hospital it was either. (After seeing the septal cartilage in the nasal tip, they said, what is this?) A large amount of costal cartilage needs to be harvested. I asked, couldn’t we reuse the ear cartilage currently inside my nose + harvest unused ear cartilage and use the two together? They said ear cartilage is too small in quantity, so costal cartilage is needed. Diced nasal bridge (crushed costal cartilage + wrapped in donated dermis + 3D mold) - to get rid of the overly slim shape and create a gentle contour before placing it The nasal tip would be left untouched as is. However, the lower part of the nose also looks slightly too slim, so they suggested a small cartilage graft above the alar area using remaining costal cartilage. I showed them a photo of Choi Hee, and they said a similar feel would be possible. I forgot to ask whether removal would be possible without damage even though the nasal bridge is probably adhered
I went for consultations because both places are known for not overdoing things, but the plans are different, and the estimates are exactly twice as much apart. I’m planning to visit three more places, but I’m already conflicted. Diced cartilage isn’t that common, the price isn’t easy either, and harvesting autologous costal cartilage feels a bit burdensome. But since I’m thinking of this as my last surgery.. If I’m going to do it anyway, I do kind of want to get rid of that chalk-like? feeling, but honestly, I had only been stressed about the step-off and the starting point of my nose. But when JY said it looked chalk-like, I started thinking maybe my nose looks too slim..? That kind of thing. It does seem like a nose with a little more volume (not nose height, but how it looks from the front) would look more natural, but since the price is double, I’m conflicted. Is there anyone here similar to me?
