Like with most doctors, it seems like the consultation style is that the more questions you ask, the more detailed the explanations are; otherwise, it moves along pretty quickly. They said the shape of my bone structure is such that the area beside my nose is much more sunken than average, so they recommended a 7-8 mm implant. I had read in a community that if you put in anything over 6 mm, the chances of bone erosion and canine tooth pain are really high, so I was taken aback as soon as I heard 7-8 mm. When I asked if many people get implants as high as mine, they said that if someone goes really high, they even go up to 9 mm. From the surgeon’s perspective, they said an implant thicker than 6 mm is actually easier to operate with.. When I asked why, they said that if the silicone is thin, it feels floppy when being inserted, so it is harder to control. Even before I could properly bring up side effects, they confidently said there would be absolutely no nasal widening, monkey-like appearance, or lengthening of the philtrum. Personally, I was most worried about bone erosion, so when I asked about it, they emphasized again that during surgery they lift the area, check it, carve the implant to precisely match the shape of my bone, and insert it, so erosion absolutely would not occur. I said that since my cheekbones are vertically long (= my midface is long), I was worried that if I got implants, my already long cheekbone area might look like it extends all the way to where the implant is placed. They said they had already considered that possibility, which is why they recommended 7-8 mm. To fill it completely would be 9 mm, but if I put in 9 mm, there is a chance it could look the way I described.. I had actually seen someone who had paranasal augmentation say later that their cheeks looked saggy, so at this point my worries about whether to get the surgery at all became much bigger. The consultant did not push very hard, but the consultation did feel a bit cursory. They put a cotton swab inside my mouth and lifted the area to be operated on to show me what it might feel like after surgery, and I thought that if it turned out exactly like that, it would be really nice. Because the shape of my bone structure itself is quite sunken beside the nose, I did feel like I clearly had an indication for it. I left without putting down a deposit. This was my first in-person consultation for paranasal augmentation.
