Consultation notes: I originally had a broad nose, and a long time ago I had silicone + septal cartilage + osteotomy.
The doctor said that with revision rhinoplasty, once you open it up, it almost never turns out exactly as expected. The capsule has to be removed too, otherwise fluid can accumulate periodically. I was told it would be best to leave the septum in place and only trim the upper part a little to adjust the height. (Hearing from another clinic too that it would be better to leave the septum helped me make my decision.)
