Hello. I had nose surgery 6 times ㅜ_ㅜ First, basically, it was done with an open approach using silicone and ear cartilage. There wasn't any particular problem, but I didn't like the shape, so I had the first removal. As everyone probably knows, before surgery, you think the person in the mirror is how you look.. but in reality left and right are reversed, and photos are the face other people see.. Also, it's rare to press the nasal bone with your hand to see if it is smooth, or to touch the nasal dorsum from the front rather than the 45-degree nasal line. Actually, among my friends who have not had surgery, even when I touched the nasal dorsum of friends whose noses looked quite smooth, it was slightly bumpy, so I realized, "Ah, if silicone is inserted, this part has no choice but to be shaved." First, if you have rhinoplasty, except for the closed approach, there is manipulation of the cartilage. To narrow the width of the nasal tip, they raise the angle of the nasal tip upward and narrow the width of the nasal tip.. In addition, to reinforce the leg portion of the nasal tip cartilage (alar cartilage), they may place a strut, or if the nasal tip is short, they may place a septal extension strut, and generally, cartilage is often added on top of the nasal tip.. First, the main point is that your priority is probably removal and returning to your original nose. The nasal dorsum is almost nine times out of ten trimmed, shaved, or cut away. That is decided depending on how smooth the nose was before surgery. On the face, a 1 mm difference is extremely large and can completely change the facial image, so the impact is big. If you have surgery on the nasal dorsum, even 1 mm may be shaved or cut, and if you remove it, the roof part of the △ will disappear. So people whose nasal dorsum base width was originally wide and only the roof part of the nasal bone was narrow become really spread out after removal.. It becomes extremely wide. In these cases, the base part absolutely has to be closed with osteotomy, and as you know, the roof part cannot be narrowed with osteotomy, so the cut roof part has to be restored even forcibly. The best options are bone source or silicone, which have the same hardness as bone. But would someone who distrusts plastic surgery want to cut their own nasal bone and use artificial silicone................... The best material is one that is firm like the original nasal dorsum and has absolutely no inflammation. These are the methods I have tried.. 1. Finely chopping ear cartilage, wrapping it in Surgicel, and placing it on the cut nasal dorsum -The drawback is that absorption is very irregular, and when touched by hand, it feels very rough, like there is acne inside. There is no refined shape, and it cannot be restored to an 11-line contour.. In other words, only the degree of lowered height from the side is restored; from the front, the slim △ roof shape cannot be restored. 2. Fat or autologous dermis Placing it on the cut nasal dorsum -The drawback is that I think the absorption rate is better than method 1, and the height can be increased from the side, but from the front, there is absolutely no effect of showing the original 11-shaped nasal bone lines and making the nasal bridge look narrow. It feels like a thin person wearing thick clothes, so it feels bulkyㅣ.. In other words, from the front, it can actually remain looking worse. 3. Filling it with costal cartilage or septal cartilage First, septal cartilage cannot fill the nasal dorsum as much as the amount cut away. Given the characteristics of Koreans, unless someone has a really high nose, and I don't think it is right to forcefully correct it to the point of putting strain on the nose.. (Later, if you suffer trauma, the nose could collapse.) Costal cartilage has a tendency as a material to warp, and this also should not be inserted as a whole piece but finely chopped like ear cartilage. Even if it raises the height from the side, it cannot restore the 11-line appearance from the front. 4. Filler As you know, it is not permanent, so it is not economically effective. Also, it cannot bring out the 11-line shape from the front. In other words.. if you want to catch both rabbits, front and side, what safe material has a firm feeling like the original nasal bone? For people who have never had surgery, the periosteal layer has not been stretched, so dermis or fat is not a big problem. But after removal, it is like wearing stretched-out clothes, so of course it will spread... The easiest way to catch both rabbits is silicone. It can restore the side and front feel to the original state. But if you distrust it, it is better not to do it. 5. Bone source The safest material that can restore everything from the front and side is a material called bone source.. This is used in orthopedics to fill depressed areas of the skull or places where bone is lacking. It is 100% the same material as bone, so there is no inflammation, and it is a material that matches our nasal bone.. But as far as I know, domestically, there is about 1 person?? who performs nose surgery with this method. Others often use it for back-of-head or forehead contouring, but people who do nose surgery with this method are extremely rare. Using this material is the safest. Some celebrities are said to have nose surgery with this method a lot.. And as for restoration of the nasal tip, if no strut was placed, there is no issue with cartilage that was cut and tied, because it can be untied again and fixed in its original position with cartilage repositioning. However, if a strut was placed at the nasal tip using costal cartilage or a hard material, complete removal may not be possible. The lower leg portion of the nasal tip and the costal cartilage or septal cartilage can wear against each other, and if removed, the leg portion may become thin, causing what is commonly called nasal tip collapse.. (drooping phenomenon..) Therefore, even if everything is taken out, cartilage repositioning has to be done again. If there is a lot of damage to the leg portion of the alar cartilage, even a little of the strut has to be left behind. I had a total of 6 procedures and finally restored it with bone source. For the nasal dorsum, after trying all three methods of ear cartilage-autologous dermis-costal cartilage-fat, the result was that all of them were worse than silicone. Mine is not silicone, so there is no inflammation, I can make a pig nose, and it feels the same as nasal bone. But I don't know who in Korea uses this method.. Everyone, if you really want to restore it to the original, solve the nasal dorsum with silicone or bone source, and for the nasal tip, cartilage repositioning is essential..
