People with bulbous noses should definitely consider columellar extension (the author highly recommends it) I'm so glad I got surgery before getting any older!! The downsides are the autologous rib cartilage scar (still, I had the incision under the breast, so if I wear underwear it doesn't show, huge win), and my nostrils changed into 110-volt outlets so I can't really pick my nose, and I can't rub my nose.
