Since deciding to undergo surgery for a bulbous nose, I had felt resistant not only to external implants at the nasal tip but also to grafting autologous cartilage. The reason was that patients with bulbous noses already have large noses to begin with, and if you put something else into an already large nose, it could make the nose even bigger.
In the end, I had decided to have only cartilage repositioning and cartilage tying, but I became concerned about the fact that the surgery would be done with an open approach and ended up giving up on having the surgery after all. I could not accept leaving a scar in my columella, so it was an unavoidable choice.
Although I ultimately decided not to have the surgery, I was deeply moved by how the consultant kept taking notes throughout the consultation and tried to reflect the patient’s requests as much as possible.
