Review of revision surgery for contracted nose, bulbous nose, and fist-like nose (autologous dermis)

Before/after I had surgery with silicone 19 years ago. At that time, programs where candidates selected through auditions received plastic surgery were popular, and I went to a famous doctor who had appeared on TV and had surgery. I should have been suspicious when the consultation ended in 5 minutes, but back then I had no information and was too young to judge, so because of a wrong choice I lived for 19 years with a nose that looked obviously operated on to anyone. At some point, the tip of my nose lifted and looked pinched, my nostrils became prominent even from the front, cartilage showed through at the nasal tip, and the tip stretched as if the aggressive silicone were about to protrude, with indentations on both sides as contraction progressed. I went to an ENT clinic, and they asked if I had ever had nose surgery because of an accident, which made me so angry and sad. People who have experienced a contracted nose will know how much emotional suffering it causes. With my return to work coming next year, I felt I should stop putting it off and resolve it, so I registered on a plastic surgery community and gathered information through research. Hoping this would be my last surgery, I chose autologous dermis because it is said to have very few side effects and look natural. Autologous dermis seems to be handled well by someone with long experience, so I went all the way to Suwon for a consultation because they were said to be famous for autologous dermis rhinoplasty. The doctor specialized in revision rhinoplasty and seemed to have know-how, so I trusted them. I went thinking I would just get a light consultation, but I felt this was the place, so I set a surgery date and came back. Rather than forcing the surgery, they divided it into stages 1 and 2, managed inflammation, and checked the condition of my nose, so it felt different from a factory-style clinic. During the consultation, they didn’t show before-and-after photos of other cases or show an expected appearance with Photoshop, so right before surgery I had a vague anxiety. In an additional consultation, I also brought up osteotomy and alar reduction, but they said those did not fit my case, and they spoke with confidence about what the shape would be like after surgery. As we talked, I realized the director was catching even the subtle details, so I felt reassured. When I had the consultation, I only hoped my nose would become normal so people wouldn’t stare, but after doing it, I keep thinking, why didn’t I have the courage earlier, I should have done it sooner. Even after COVID ended, I couldn’t take off my mask, but after the surgery, it feels like my personality and my mind are healing too. I’m satisfied just to have resolved the contracted nose, and I’m happy to have gained a comfortable nose. It hasn’t engrafted yet, so the swelling has not gone down, but overall the line seems to be settling beautifully, so I’m satisfied.
