I had philtrum reduction surgery about 3 years ago. At the time, they said they would use dissolvable sutures and operate inside the nostrils, so I thought the scars would not show... But what do you know, I have no columellar base tissue at all, huge nostrils, and the scars are clearly visible too The philtrum definitely became shorter, but if I were to do it again, I think I would just go somewhere that excises below the columellar base.
