I agree with all the problems and points for improvement, but the reason my surgical expectation level is 3 is that there was no inflammation, yet they kept presuming in advance that there would be. My alar cartilage is very collapsed, but they said they would reuse the existing autologous costal cartilage, which will definitely be insufficient. They also suggested using AlloDerm, which will be absorbed over time anyway.
And decisively, I showed them photos of my desired nose line, with a rounded nasal tip, but they said it in a way that suggested it couldn’t be done. They seem honest and skilled.
