After the CT scan and waiting about ten minutes, the consultation started. I had watched YouTube videos before going, so I had a rough sense of what style the director had. I felt that he was kinder than in the videos and tended to examine things very thoroughly. While alternating between the images taken, he showed me what my current nose shape looked like and how he would set the nose design by drawing with the mouse, and he also showed me by drawing with a pencil on paper. I could tell he was trying to explain it as easily as possible for me to understand, and while talking, he would ask whether I understood. When he set the design, he asked whether I liked it there or wanted it higher, and if he thought it was excessive, he tended to say firmly right away that it was not possible. He said the surgery would be done through an open approach under the nose, and that putting in 3-4 mm silicone would be the most appropriate for my nose. However, because my septal cartilage is insufficient, he recommended using rib cartilage, and said that both donated rib cartilage and autologous costal cartilage can provide firm functional support, but donated cartilage is literally someone else's, while autologous cartilage is my own, though it creates another surgical site, so that is something I need to decide. And he said having a bulbous nose doesn't automatically mean you need alar reduction, and in my case, I don't need it! Thank goodness They have CCTV, and a 1-year free AS is available.

I think it would be better for someone who doesn't know much about surgery, or who has trouble making decisions. If someone has very firm opinions, they might not match well with the director.. Still, I think listening to the director is important too, so I listened to the nose design as he set it, and here too, the director said 4 mm silicone would be the maximum. And he was like, what alar reduction, you don't need it, and said that with silicone, most bulbous noses improve. He said there was no need to overdo anything more. I'm really relieved.. I was worried this would make recovery slower.. He likewise said rib cartilage would need to be used, but he also mentioned that autologous dermis could possibly be used, so I heard an explanation about that too before leaving. It was my first time hearing about dermis, so it was a bit hard to understand I had even paid a consultation fee, but the consultation ended faster than I expected, so that was a little disappointing. Still, because the director spoke in a tone that mixed gentleness with firmness, it was also a consultation that helped me organize my thoughts.
