If you tell the director what part you want, they try to do as much as possible. I went with a promotion that included a retouch, and with lip filler, a retouch feels absolutely necessary. It was hard to be satisfied with the design from just one procedure. When injecting the filler, they did it well with very little pain, and there were no side effects.
