They look at the nose in really fine detail. This is the first hospital I've heard say the columella is too thick. So they said that if you do alar rim lifting, the nostrils will be small but the columella will still be thick, so the columella also needs to be thinned. There may be scarring, and they said they would only make about three needle puncture incisions. There is no scar laser. But they don't do sleep sedation here. It's local anesthesia only. Also, the director is really funny.
