I went for a consultation to look into lower eyelid surgery for my mom, and they mentioned lower blepharoplasty, which surprised me, but after hearing the reason I was convinced. If it's just fat bulging out, then fat repositioning is enough, but (for younger people) if the skin has sagged, they said it should be lower blepharoplasty that lifts the skin too. They were very firm that if I wanted lower eyelid surgery, they couldn't do that, and explained in detail the side effects of fat removal and grafting, and the problems with some dermatology procedures
